Michael Moore’s Newest movie on Capitalism Affects us!

The two main points that I took out of Michael Moore’s movie is that

1. We handed over a Trillion on the military because we were scared. We handed over nearly a trillion on the bailout because we were scared.

We are NOT handing out what we need on healthcare because we are NOT able able to make the same case for fear…. it is not as frightening and immediate as a military attack or economic fears. Yet, the reality is FAR worse. The calm nature of the physician, the measured tones, have made us losers in in political realm where fear of disaster and failure is what makes people move.

We MUST sell healthcare reform as 1. A military attack on the country.   2.The GREATEST source of economic ruin for the country.

Then people will move.

Second point of Michael Moore’s movie.

The BEST and BRIGHTEST have been kidnapped into the Finance industry, NOT the cure of disease. This is a central theme of “Routine Miracles” . Time to get the BEST back  into medicine.

As a psychiatrist, I must disagreee with you: certainly those who oppose a public option are, in my opinion, uninformed, ignorant, meanspirited and hateful. However, to equate these folks with patients who are truly mentally ill and are the products of de-institutionalization is, I feel grossly unfair to and prejudiced against the mentally ill. Just because some of them get bussed to a demonstration doesn’t mean most of our (my) patients are among them.

As a psychiatrist, I must disagreee with you: certainly those who oppose a public option are, in my opinion, uninformed, ignorant, meanspirited and hateful. Five Days of Presentations in California Just Done

From this past sunday,  until about 11 hours ago,  I was in California making a number of presentations on ‘Routine Miracles”.  University of San Diego, University of Southern California, UCLA, Family Practice, Kaiser Sunset Hospital, The Block Radio (http://www.theblockradio.com/) , Zocalo Square literary Magazine, two Kaplan Centers.

It’s not so easy to stay positive in the face of so many fixed, negative ideas. There is a nearly universal belief in the minds of the students and the doctors that the best days in Medicine are for some reason in the past. The same fear based psychology. I see no evidence of a ground swell of excitement about the burning issues of Health Care Reform. I see no evidence that the near certitude that there is to be the largest leap forward in healthcare coverage in forty years is about to happen and that the country will be living up better to its ideals.

I See little evidence of leadership trickling down to the students and the residents that we are in the incipient age of a Greater Civilization that casts a finer value on all our citizens and that we are to have the greatest explosion in medical education in a century.

Not one person has greeted me with a word showing that the Research Budget in the United States has just been raised to the most generous in the history of the world.

Where then, are our hearts?   Where are the ideals that gave you the impulse to enter medicine? How much longer will your life be tormented by fear?

We are entering a golden age of medicine. The Miraculous is occuring.

This morning I was awoken by my alarm clock powered by electricity generated by the public power monopoly regulated by the U.S. Department of Energy.

I then took a shower in the clean water provided by a municipal water utility.

After that, I turned on the TV to one of the FCC-regulated channels to see what the National Weather Service of the National Oceanographic and Atmospheric Administration determined the weather was going to be like, using satellites designed, built, and launched by the National Aeronautics and Space Administration.

I watched this while eating my breakfast of U.S. Department of Agriculture-inspected food and taking the drugs which have been determined as safe by the U.S. Food and Drug Administration.

At the appropriate time, as regulated by the U.S. Congress and kept accurate by the National Institute of Standards and Technology and the U.S. Naval Observatory, I get into my National Highway Traffic Safety Administration-approved automobile and set out to work on the roads build by the local, state, and federal Departments of Transportation, possibly stopping to purchase additional fuel of a quality level
determined by the Environmental Protection Agency, using legal tender issued by the Federal Reserve Bank.

On the way out the door I deposit any mail I have to be sent out via the U.S. Postal Service and drop the kids off at the public school.

After spending another day not being maimed or killed at work thanks to the workplace regulations imposed by the Department of Labor and the Occupational Safety and Health administration, enjoying another two meals which again do not kill me because of the USDA, I drive my NHTSA car back home on the DOT roads, to my house which has not burned down in my absence because of the state and local building codes and Fire Marshal’s inspection, and which has not been plundered of all its
valuables thanks to the local police department.

And then I log on to the internet — which was developed by the Defense Advanced Research Projects Administration — and post on Freerepublic.com and Fox News forums about how SOCIALISM in medicine is BAD because the government can’t do anything right.

In “Routine Miracles” I  talk about the miracle of a 90% reduction in in-patient psychiatric hospitalizations.  I now know where many of them are. They were on the march on washington against a public option a few weeks ago.

Today I am truly grateful for de-institutionalization. I am particularly struck with the ‘Bury Obamacare with the Kennedy’s” which is definitely the most vile thing I have heard this year, and I would not have understood how truly ILL the people are who oppose healthcare reform are, how uninformed, ignorant, meanspirited and hateful.

Keep the public Option alive

I strongly urge you to call the senate finance committee to keep the public option in financing alive.  I think this is a strong incentive to competition and is in the best interest of the American People.

It is the way of the Post Office, Amtrak and many other areas in which we live. Private insurance is a business.  A business is based on profit, in which THEY, the owners and shareholders are to keep as much as possible, and give you as little as possible.

Why do you think the insurance industry is so scared of this?

HIV and pregnancy Question

The criteria for starting anti-retrovirals in pregnancy are

1. If the CD4 is LOW (<350) or the Viral load is elevated, then start THREE drugs IMMEDIATELY

2. If the CD4 is HIGH (>350) and the viral load is LOW, then start medications in the 2nd and 3rd trimester and stop after delivery

3. C-Section is ONLY used if the viral load is above ONE THOUSAND. this is a special cut off just for preventing perinatal transmission of HIV.
